What do I need to bring to my first appointment?Driver's license, insurance, new patient paperwork (if this hasn't been completed through online check in), CPAP machine - if you have one.

How can I schedule a sleep test?First, you will need to schedule a consult with one of our providers at Quality Sleep & Neurology.

How do I get a referral for testing?For sleep medicine, you can self refer. If your insurance requires a prior authorization, such as Blue Care network, your primary care provider need to send a referral to our office. For Pediatric Neurology: We must have a referral sent to our office from your primary care physician. You can get a referral from your PCP through appointment, or if your insurance permits, you can schedule an appointment directly with office. Call 989 558-0773 for this option, Monday - Thursday between 8 a.m. - 4:30 p.m. EST., phones are shut off daily from 11:30 a.m. - 1 p.m. for lunch and Fridays between 8 a.m. - 11:30 a.m. Providers looking to refer your patient, please use "Referral to Sleep Medicine and select Quality Sleep and Neurology" in Epic or fax number 989-270-1274 or give us a call at 989-558-0773.

I took a sleep test, but the results were negative. Should I still schedule an appointment with Quality Sleep & Neurology?Yes, it is highly recommended to schedule an appointment with Quality Sleep & Neurology even if your sleep test results were negative. Negative or inconclusive results does not necessarily mean that you have quality sleep health. Our providers can help identify underlying issues that.
